To fill out this form, follow these steps:
1. Type in the district name. The district name will appear on all three copies of the Oath page.
2. Tab to the next field. This field is for the date the Oath will be signed. This form can be printed with or without the date. If entering the date, use the mmm/dd/yyyy format. For example, Nov. 15, 2012.
